HWY 4 Now Open After Fatal Collision – Name Withheld Pending Next Of Kin Notification

(MIDDLESEX CENTRE, ON) – On April 15, 2017, at approximately 11:54 a.m., the Middlesex County Ontario Provincial (OPP), Middlesex Centre Fire Services, and Middlesex-London Emergency Medical Services (EMS), responded to a two vehicle collision on Richmond Street.

A black Dodge minivan which was travelling northbound on Richmond Street north of Eight Mile Road struck the rear of a white Ford cube van, which was also northbound and stopped on the roadway to turn into a private drive.

The 54-year-old male driver and lone occupant of the black van had to be extricated from the vehicle. He was transported to hospital by ambulance with life threatening injuries, and has since died. His identity is being with-held pending notification of next of kin.

The two occupants of the cube van were not injured.

The OPP Technical Traffic Collision Investigators (TTCI) attended the scene to assist with the investigation. As a result, Richmond Street (Highway 4) was closed from Eight Mile Road to Nine Mile Road until approximately 7:30 p.m. and has been re-opened.

This incident remains under investigation.
